Dental material "Belodez" based on a stabilized solution of sodium hypochlorite

PURPOSE

Dental material "Belodez" is intended for:

3% solution (gel) for medicinal treatment of root canals as a bactericidal, hemostatic and bleaching agent, for chemical expansion of canals in combination with EDTA solutions, and also for disinfection of the tooth cavity.

5.2% and 10% solution for treating canals with pulp diathermocoagulation or pulp coagulation with strong medicinal agents, as well as for disinfecting gutta-percha and metal pins, orthopedic and orthodontic structures and products before their installation in the oral cavity.

COMPOSITION AND MAIN PROPERTIES

Belodez is a stabilized sodium hypochlorite solution (gel) that dissolves the organic base of dentin, including both living (chemically fixed) and necrotic tissue. Its gel form allows it to be successfully used for antiseptic treatment of maxillary cavities, as it is sufficiently viscous, fluid, and gentle on the mucous membrane.

"Belodez" exhibits bactericidal action against all gram-positive and gram-negative microorganisms, fungi, and viruses without apical cytotoxicity. When treated with sodium hypochlorite, the hydrogen of the amino groups (-HN-) in protein molecules is replaced by chlorine (-ClN-), forming chloramine, which plays a key role in antimicrobial activity. The bactericidal action of these products is also due to their alkaline properties (pH 10.0-12.0).

The range of hypochlorite's action depends on its concentration in the material. At concentrations of up to 1%, only necrotic tissue, decay, and pus are dissolved. Sodium hypochlorite at higher concentrations affects living tissue, and the higher the concentration of the solution, the greater the damage to vital cells.

Sodium hypochlorite (3%) is used to dissolve residual pulp after vital extirpation. Dissolution of mummified pulp remnants after devitalization with strong medications or after thermocoagulation is more effective with sodium hypochlorite (5.2%) and 10%). However, it should be noted that high concentrations of sodium hypochlorite can destroy tooth tissue more quickly than microorganisms. Therefore, in a clinical setting, when treating root canals, the exposure time of concentrated solutions (5.2% and 10%) should be strictly limited (no more than 60 seconds).

When working with sodium hypochlorite, it is necessary to take into account the dependence of the solution's effectiveness on temperature: at a temperature of 37ºC, the activity and rate of disinfection of low-concentration solutions (0.5-1%) is not inferior to 3-5.2% solutions.

To remove the smear layer (created by instrumentation), sodium hypochlorite is used with EDTA-containing preparations, which bind calcium in the dentin, creating a loose structure in the canal. Combined treatment with sodium hypochlorite and EDTA-based preparations (EndoZhi No. 2, EndoGel) effectively cleans and widens infected canals, and significantly improves the adhesion of filling materials to the canal walls.

By diluting a 10% liquid with distilled water in a ratio of 1:2, a universal solution (3.3%) can be obtained.
